“The Access to Care program is designed to identify uninsured children and connect them to an appropriate and affordable source of quality health care services in the community... has successfully connected more than 28,000 children to a source of health care services.”
“The Prevention program...provide[s] a variety of health education programs designed to increase the knowledge and improve healthy choices in the area of asthma prevention/management, nutrition and physical activity. More than 90,000 children, parents and educators have participated in PHK Prevention activities.”
“...Dr. Clark has been instrumental in helping develop another community focused program...called the Home Tomorrow program. This program resulted in a drastic reduction in hospital admissions for this vulnerable population of children and has been used as a template by other healthcare systems to develop similar programs in their region.”
